This is a scene from the episode"Dreamscapers". For most of you, this would have been the first time you would have ever seen him(because you would not have known that he existed back then). Bill Cipher is actually a powerful mind demon and most probably tricked the author of the journals to trust him. Journal 3 had a page where the author writes about him and then scribbles the page with something red and the entire two pages are splattered with red. 

He has actually been pictured on almost every episode. even the first episode "Tourist Trapped".
